Physician Terms

Plain-Language Summary

Core Bio Care works alongside licensed healthcare providers to support patient access, coordination, and follow-through. Core Bio Care does not provide medical care or interfere with clinical judgment. Physicians remain fully responsible for all medical decisions related to their patients.

Physician Terms

These Physician Terms (“Terms”) apply to licensed healthcare providers (“Physicians” or “Providers”) who interact with Core Bio Care in connection with patient services, referrals, coordination, specimen collection support, or related activities.

By engaging with Core Bio Care, Physicians acknowledge and agree to these Terms.

Role of Core Bio Care

Core Bio Care provides non-clinical support services that may include:

  • Care navigation and coordination

  • Telehealth facilitation support

  • Mobile access and logistical assistance

  • Specimen collection support where permitted

  • Administrative and communication support

Core Bio Care does not provide medical care, diagnosis, treatment, or clinical decision-making.

Physician Responsibility

Physicians retain full clinical responsibility for:

  • Determining medical necessity

  • Ordering tests or services

  • Diagnosing conditions

  • Interpreting results

  • Communicating medical findings and treatment plans

  • Providing follow-up care

Nothing in Core Bio Care’s services alters, transfers, or reduces a Physician’s professional responsibilities to their patients.

No Interference with Clinical Judgment

Core Bio Care does not influence, direct, or interfere with clinical judgment. Physicians maintain independent decision-making authority at all times.

Core Bio Care’s role is limited to facilitating access, coordination, and understanding of care as directed by the Physician or authorized laboratory.

Orders, Referrals & Coordination

Physicians may interact with Core Bio Care through multiple pathways, including:

  • Placing orders through authorized platforms

  • Coordinating services through laboratories

  • Referring patients to Core Bio Care for non-clinical support

All medical orders must originate from a licensed provider or an authorized laboratory medical director, consistent with applicable laws and regulations.

Specimen Collection & Laboratory Coordination

When specimen collection or laboratory coordination is involved:

  • Core Bio Care’s role is limited to non-clinical collection support and logistics, where permitted

  • Laboratories and/or ordering providers retain responsibility for test authorization, processing, interpretation, and reporting

  • Core Bio Care does not validate, interpret, or modify laboratory results

Patient Consent & Information Sharing

Physicians acknowledge that patients engaging with Core Bio Care provide consent in accordance with Core Bio Care’s Consent to Services Policy.

Information shared with Core Bio Care is used solely to support service coordination and operational needs and is protected under Core Bio Care’s privacy and data protection practices.

Data Protection & Confidentiality

Core Bio Care applies HIPAA-aligned safeguards to protect sensitive information and limits access to the minimum necessary to perform services. Physicians are responsible for compliance with their own professional and legal data protection obligations.

Independent Relationship

Nothing in these Terms creates:

  • An employment relationship

  • A partnership or joint venture

  • An agency relationship

Between Core Bio Care and any Physician or medical practice.

Limitation of Liability

To the fullest extent permitted by law:

  • Core Bio Care is not responsible for clinical outcomes, medical decisions, or patient treatment

  • Physicians remain solely responsible for care provided to their patients

Core Bio Care’s liability, if any, is limited to its non-clinical services as defined herein.

Modifications & Termination

Core Bio Care reserves the right to modify or discontinue coordination with any Physician or practice if:

  • Safety concerns arise

  • Operational boundaries are exceeded

  • Continued coordination would compromise service integrity

Such actions do not alter a Physician’s obligations to their patients.
